Relationship between Blackbird Health and the Medical Practices

Blackbird Health provides administrative and management services to independent provider-owned-and-operated health care practices. Blackbird Health provides these services for Children’s Integrated Center for Success, LLC d/b/a Blackbird Clinical Services, and other affiliated health care practices that use Blackbird’s business support services. Each Blackbird-branded practice is owned and operated by a licensed provider. There is no single provider of medical care called “Blackbird Health.” Each Blackbird-branded practice engages a network of licensed providers who provide clinical telehealth and in-person services. Blackbird Health does not provide health care advice or care, own, or operate the health care practices, employ or in any way supervise the providers providing health care, and control over the care provided is the sole responsibility of the independent health care practices and the providers they employ. Services and practices may vary across providers, and patients should contact the providers at "Blackbird Clinical Services" directly for all questions concerning their care
